As a key member of our nursing team, you will:
- Oversee and personalize patient care services
- Visit clients in their home to assess for services
- Supervise & support our home health aide staff in the field
- Create care plans and acquaint home care aides with new clients
- Assess new referrals for services

The ideal candidate should possess:
- An active Massachusetts RN/LPN licenses and with 2+ years experience.
- Strong clinical and computer competence
- Excellent communication skills
- A TEAM player that is self-motivated with the ability to function autonomously and manage priorities
- Demonstrated ability to problem solve and motivate others
- Home care experience a plus.
- Medicare/Certified service experience is not required.
